Community Healthcare Assistant – Community Nursing Team
Location: Segensworth, Fareham (travel across Hampshire required)

We are seeking a reliable, compassionate, and motivated Bank Community Healthcare Assistant (HCA) to join our Community Nursing Team.
This is a dynamic and rewarding role where you will support the delivery of high-quality, dignified care to patients in their own homes across Hampshire. As part of your role, you will also be responsible for driving a company vehicle to attend patient visits and support service delivery.
You will work closely with qualified nurses, assisting with both clinical and non-clinical duties, ensuring patients receive safe, effective, and person-centred care.
The service operates overnight from 20:00 to 08:00 Monday to Sunday
Key Responsibilities
· Deliver personal care with dignity and compassion
· Support nurses with wound care, palliative care and post mortem care
· Take and record patient observations (blood pressure, pulse, SpO₂, temperature, respirations, blood glucose)
· Assist with documentation and digital record keeping
· Drive safely to patient visits across Hampshire (vehicle provided)
· Keep equipment stocked and vehicle service-ready
· Support with non-clinical tasks, such as transporting supplies
· Attend visits independently where risk-assessed as appropriate
What we’re looking for
Essential:
· Experience as a Care or Healthcare Assistant, Support Worker or similar role
· Full UK clean driving licence
· Basic clinical skills and confidence with personal care
· Physically fit for manual handling tasks
· A positive, compassionate and reliable approach to patient care
Desirable:
· Community care experience
· Experience in wound care, catheter care, or Palliative care
· Experience with Verification of Expected Death (VoED)
· Familiarity with digital care record systems

Pay: £13.93-£18.11 per hour
