Job Summary
We are seeking a compassionate, reliable and dedicated Care Assistant/Key Worker to join our team. This role involves providing high-quality care and support to adults with mental health conditions and learning disabilities. The successful candidate will play a vital role in promoting independence, dignity, choice, and wellbeing while supporting individuals to achieve their personal goals and live fulfilling lives. Responsibilities
- Provide person-centred support tailored to individual needs and care plans.
- Promote independence and encourage service users to develop daily living skills.
- Assist with meal planning, food preparation, and cooking.
- Support individuals with household tasks, including cleaning, laundry, and maintaining a safe living environment.
- Accompany and escort service users to medical appointments, social activities, educational opportunities, and community events.
- Conduct general health and wellbeing checks, reporting any concerns to senior staff or healthcare professionals.
- Support individuals with medication management in accordance with company policies and procedures.
- Encourage participation in recreational, educational, and social activities.
- Build positive, professional relationships with service users, their families, and other professionals.
- Maintain accurate records, care notes, and incident reports.
- Safeguard vulnerable adults and ensure their safety and welfare at all times.
- Act as a Key Worker, supporting individuals to achieve outcomes identified in their support plans and regularly reviewing progress.
- Qualifications

- Good communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Understanding of mental health and learning disabilities (preferred but not essential as training can be provided).
