Compton Care is proud to provide care and support to those living with life limiting conditions through several dedicated but also interlinked service including our inpatient unit, community care, our living well centre, and supportive care services.
What will you do?
We are looking for a proactive and organised Patient Flow Facilitator to support safe, timely and person-centred transfers of care across our Community and Virtual Ward services. This is a key role within Compton Care, working closely with patients, families, acute hospital wards, community teams, social care and other system partners to help people receive the right care, in the right place, without unnecessary delay.
The successful candidate will be visible and responsive across acute ward areas, building strong relationships with NHS colleagues and wider partners to identify patients who may benefit from Compton Care support, remove barriers to discharge and maintain effective flow. You will need to be confident working directly with multidisciplinary teams, attending board rounds or discharge discussions where required, gathering accurate information, escalating delays appropriately and supporting clear communication with patients and those important to them.
This role will suit someone who is compassionate, confident and solution-focused, with experience in health or social care and an understanding of discharge planning, patient flow or care coordination. You will need excellent communication and organisational skills, the ability to manage competing priorities and the confidence to work across organisational boundaries. As the role requires regular presence in acute hospital wards and community settings, access to transport and the ability to travel across the local health and care system are essential.
You will play an important part in supporting safe and effective transitions of care, reducing avoidable delays, improving patient experience and strengthening partnership working between Compton Care and our NHS and social care colleagues.
