Care North East is looking for an experienced, organised and compassionate Key Care Worker to oversee a group of allocated clients across our community service and extra care scheme.
This role combines hands-on care with additional responsibility for care planning, client reviews, quality assurance and communication. You will act as the main point of contact for your allocated clients, helping to ensure they receive safe, consistent and person-centred care that promotes their independence.
Key responsibilities
- Oversee the care and support provided to allocated clients.
- Develop positive, professional relationships with clients and understand their individual needs, preferences and desired outcomes.
- Create, review and maintain accurate Digital Social Care Records.
- Complete and update care plans, risk assessments and client reviews.
- Monitor whether agreed care plans and risk assessments are being followed.
- Arrange and participate in client reviews and update meetings.
- Liaise with families, representatives, health and social care professionals and internal staff.
- Audit medication records, daily care logs, reports and other care documentation.
- Identify concerns, changes in need or gaps in service delivery and ensure appropriate action is taken.
- Support care workers to understand and consistently meet each client’s needs.
- Promote dignity, choice, independence and a person-centred approach.
- Ensure documentation and working practices meet legal, regulatory and organisational standards.
- Escalate safeguarding, medication, health or service-delivery concerns appropriately.
About you
We are looking for someone who:
- Has previous experience working in adult social care.
- Understands person-centred care, safeguarding, medication and risk management.
- Is confident completing care plans, risk assessments and client reviews.
- Has strong written, organisational and communication skills.
- Can maintain accurate digital records.
- Is comfortable communicating with families and external professionals.
- Can identify concerns, take appropriate action and escalate when required.
- Is dependable, professional and able to manage competing priorities.
- Can support and guide colleagues while maintaining positive working relationships.
Qualifications
An NVQ Level 3 in Health and Social Care, or an equivalent qualification, is desirable. The successful candidate must already hold this qualification or be willing to work towards it.
You will also be required to complete all mandatory and role-specific training.

Out-of-Hours On-Call
As part of the role, you will be expected to participate in the out-of-hours on-call rota. This involves responding to urgent operational matters, providing guidance to staff and escalating concerns when necessary.
On-call duties are paid at a separate daily rate, in addition to your normal salary.
