As part of a multidisciplinary team, the post holder will be responsible for assessing care and support needs, developing person-centred care plans and risk assessments, and implementing, monitoring and evaluating these to ensure the delivery of safe, high-quality care and positive outcomes for people we support.
The post holder is expected to carry out all aspects of care and support with a high degree of autonomy, while working within Company policies and professional standards. They may also be required to demonstrate procedures, mentor and supervise SSSC-registered care staff.
The post holder will lead, coordinate and supervise the work of their designated team, promoting best practice, person-centred approaches and positive outcomes for people we support across a diverse range of needs and circumstances.
The post holder will also support the Service Manager and Deputy Service Manager with the day-to-day running of the service, undertaking delegated operational and administrative tasks as required to ensure the smooth, safe and effective delivery of care.

Main Responsibilities
- Lead, supervise and support a team of Support Workers, providing induction, mentoring, guidance and ongoing development
- Deliver and coordinate high-quality, person-centred care in line with Company policies, procedures and values, promoting independence, dignity, choice and positive outcomes for people we support
- Assess care and support needs and develop, implement and review person-centred care plans and risk assessments, ensuring they remain current and outcome-focused
- Support the Service Manager and Deputy Service Manager with the day-to-day operational running of the service, carrying out delegated duties and administrative tasks as required
- Manage your designated team and rota, ensuring effective staffing levels, mandatory training compliance and appropriate allocation of resources
- Maintain accurate care records and communicate effectively with people we support, families, professionals and the wider multidisciplinary team
- Ensure compliance with Company policies, regulatory standards, safeguarding, health and safety, and all relevant legislation
- Support staff performance through supervision, coaching, wellbeing conversations and constructive feedback
- Participate in audits, quality assurance and continuous improvement activities
- Act as a positive role model, demonstrating the Company’s values and promoting a supportive, inclusive team culture
Qualifications and Experience
- Full UK driving licence
- Valid SSSC Registration (or willingness to obtain)
- SVQ Level 3 in Health Social Care (or willingness to work towards)
- Willingness to participate in the on-call rota
- Previous experience in a health or social care setting, with supervisory or leadership experience desirable
- Experience of assessing care needs, developing care plans and risk assessments, and supporting adults with a range of support needs
- Strong communication, organisational and leadership skills
- Basic IT skills and evidence of ongoing professional development
- Flexibility to work a varied shift pattern, including 12-hour day or night shifts
