Overview
We are seeking a dedicated and compassionate General Practice Nurse to join our healthcare team. The successful candidate will play a vital role in delivering high-quality patient care within a busy general practice setting. Monday - Friday 37 hours a week
Responsibilities
1. To screen patients for disease risk factors, and early diagnosis of illness
2. Provide a first point of contact for patients requiring nursing care
3. Deliver routine nursing care including annual review of long term conditions
4. Administer immunisations (routine, childhood, and travel where appropriate)
5. Carry out cervical cytology and women’s health services
6. Promote health and wellbeing through patient education and lifestyle advice
7. Assess patient needs and deliver care based on evidence-based practice
8. Request and interpret pathology tests within scope of practice
9. Maintain accurate clinical records and ensure appropriate coding
10. Manage and update chronic disease registers
11. Recognise and respond to clinical emergencies
12. Refer patients appropriately to GPs or other healthcare professionals
13. Support infection prevention and control procedures
14. Participate in MDT meetings and practice development activities
15. Provide home visits for those who are unable to attend the surgery
16. Assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
17. Maintain accurate clinical records using ARDENS templates
18. Ensure SNOMED codes are used effectively
19. Maintain chronic disease registers
20. Review patches inbox daily
21. Ensure S1 TASKS are dealt with promptly
22. Ensure clinical waste is removed from clinical areas and sharps bins replaced in accordance with the practice IPC policy
23. Assist in the achievement of QOF targets and local contract requirements
24. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
25. Support the clinical team with all safeguarding matters, in accordance with local and national policies
26. Deliver opportunistic health promotion where appropriate
Secondary Responsibilities
In addition to the primary responsibilities, the practice nurse may be requested to:
a. Partake in clinical audit, effectively utilising the audit cycle
b. Monitor clinical equipment and cold chain processes including audit of fridge temperatures
c. Support members of the nursing team providing guidance when necessary
d. Participate in local initiatives to enhance service delivery and patient care
e. Participate in the teaching and supervision of nurses and doctors as appropriate and when requested
f. Maintain stock and order supplies as needed
g. Continually review clinical practices, responding to national policies and initiatives where appropriate
h. Participate in the review of significant and near-miss events applying a structured approach
i. Develop and review audit protocols and process for the practice, effectively utilising the audit cycle.
j. To maintain a professional portfolio which reflects personal development and to attend clinical supervision
Qualifications
- Registered General Nurse (RGN) qualification recognised in the UK.
- Post graduate diploma or degree (Primary Care)
- Proven experience in a primary care or general practice setting is desirable.
- Strong knowledge of anatomy, physiology, and common medical conditions.
- Competent in medication administration and understanding of relevant clinical guidelines.
- Excellent communication skills with the ability to provide empathetic patient support.
- Organised with the ability to prioritise tasks efficiently in a fast-paced environment.
