About the Role
Based at our Heswall Head Office, you’ll play a key role in coordinating personalised care services, supporting both clients and care staff to ensure safe, effective, and responsive care delivery.
This is a busy and rewarding role that requires excellent organisational skills, the ability to prioritise workloads, and confidence working under pressure.
The role includes on-call rota duties:
· One evening per week
· One weekend in five (with days off in lieu during the week)
A good knowledge of the Wirral area is essential.
Key Responsibilities
· Coordinate and deliver person-centred care in line with company policies, procedures, and regulatory requirements.
· Create and manage weekly staff rotas to ensure continuity of care and full service delivery.
· Complete assessments for prospective clients to identify individual care needs.
· Carry out regular reviews and reassessments of existing care packages.
· Develop and update person-centred care plans.
· Maintain and update Trusted Assessor portal
· Build positive relationships with clients, families, healthcare professionals, and external agencies.
· Carry out spot checks, staff supervisions, and performance appraisals.
· Maintain accurate records and ensure effective communication between the care team and management.
· Deliver excellent customer service to clients, colleagues, and external stakeholders.
Essential Requirements
· Minimum of 2 years’ experience within the Domiciliary Care or Supported Living sector.
· Level 3 NVQ/Diploma in Health & Social Care.
· Willingness to work towards a Level 4 NVQ/Diploma in Health & Social Care or equivalent
· Full UK driving licence with access to your own vehicle.
· Flexibility to participate in the out-of-hours on-call rota.
· Good IT skills, including Microsoft Office.
· Excellent knowledge of the Wirral area.
Desirable Experience
· Previous experience in a Senior role within the Health & Social Care sector
· Experience using rostering systems, particularly PASSforCare.
· Experience supervising or managing staff.
