About the role
We are looking for a reliable, compassionate and flexible Personal Assistant / Support Worker to provide practical and social support to an individual living with long-term health conditions.
The role is about helping the person maintain their independence and continue to enjoy their home, garden, social life and day-to-day activities. The successful candidate will support with tasks that the individual would do themselves if they were able to, while respecting their choices, routines and independence.
Energy management is particularly important in this role, so the ability to recognise when someone needs to rest is essential.
About the person
The person enjoys spending time in the garden, caring for plants, spending time with their dogs, having friends and family visit, and getting out to the shops when well enough. They enjoy friendly conversation and a coffee, although they can tire easily.
They live with a number of long-term health conditions which can affect energy levels, mobility and ability to carry out everyday activities. Full details will be discussed with shortlisted candidates.
Main responsibilities
Duties will vary from day to day depending on the individual's needs and may include:
Social and community support
- Accompanying the individual to medical appointments and taking notes where required.
- Providing advocacy and helping communicate their wishes when needed.
- Supporting time spent in the garden.
- Assisting with trips to the shops and other outings.
- Assisting with a wheelchair when out of the house.
- Helping with emails, telephone calls and other communication when required.
Personal support
- Supporting preparation for bathing and providing occasional assistance with dressing.
- Prompting medication as required.
- Preparing food and drinks.
- Supporting with diabetes-related foot checks.
- Applying moisturiser and prescribed topical creams as directed.
- Assisting with hair washing and drying when required.
- Encouraging appropriate use of inhalers when experiencing breathlessness.
- Collecting prescriptions.
- Monitoring and reporting changes in pain or symptoms.
- Helping arrange and attend appointments when necessary.
- Providing steadying support when the individual is unbalanced.
- Encouraging regular rest periods and appropriate energy management.
- Supporting good hygiene.
Domestic support
- Planning meals and food requirements for the week.
- Grocery shopping and meal preparation.
- Cooking and serving nutritious meals and snacks.
- General household cleaning and washing up.
- Changing bed linen.
- Laundry and ironing.
- Gardening and maintaining the home environment.
Please note: Duties may vary from day to day depending on the individual's needs and energy levels.
Hours
Current shifts include:
- Morning: approximately 9:15am–12:15pm
- Afternoon: approximately 3:30pm–5:30pm, or around this time to ensure the evening meal is covered.
There is some flexibility around these times, and additional hours may be available for medical appointments and other specific needs.
The morning shift generally covers breakfast and lunch, while the afternoon shift covers the evening meal and related support.

Essential requirements
- Full driving licence.
- Enhanced DBS check.
- Access to a suitable car with appropriate insurance for the role.

- Patience, empathy and a supportive approach.
Desirable
- Basic computer skills.
- Good communication skills.
- Comfortable around dogs.
- A naturally good listener.
