We are looking for a caring and organised Care Coordinator to support patients in accessing the healthcare services they need.
Key Responsibilities:
- Coordinate annual health and wellbeing reviews for patients with learning disabilities and severe mental illness.
- Provide brief interventions for patients with non-diabetic hypoglycaemia and refer eligible patients to the NHS Diabetes Prevention Programme.
- Identify and contact patients who may benefit from NHS weight management services, making referrals where appropriate.
- Support the delivery of COVID-19 vaccination programmes, including coordinating services for housebound patients and assisting at vaccination hubs.
- Contact patients who have not attended cancer screening appointments, offering support and arranging appointments to improve screening uptake.
- Support immunisation campaigns by inviting eligible patients to programmes such as Polio catch-up, MMR catch-up, Pneumococcal, and Shingles vaccinations.
- Assist in the delivery of care for patients with long-term health conditions.
- Support patients living with cancer by coordinating care reviews and ensuring appropriate follow-up.
