To provide high-quality, person-centred support to women with severe and enduring mental health needs, enabling them to maximise their independence, develop daily living skills, achieve their recovery goals and safely participate in their local community. Rehabilitation Workers work collaboratively with residents, families, healthcare professionals and the wider multidisciplinary team to promote recovery, wellbeing and social inclusion.
Key Responsibilities
Person-Centred Support
- Build positive, therapeutic relationships based on trust, dignity and respect.
- Support residents to achieve the goals identified within their support and recovery plans.
- Encourage independence while providing appropriate levels of support.
- Promote choice, control and positive risk-taking in line with individual care plans.
Mental Health Rehabilitation
- Support individuals experiencing complex mental health conditions, including psychosis, schizophrenia, bipolar disorder, personality disorder and trauma-related conditions.
- Encourage engagement with treatment, medication and therapeutic interventions.
- Observe and accurately report changes in mental or physical health.
- Respond appropriately during periods of distress or crisis using de-escalation techniques.
Daily Living Skills
Support residents to develop and maintain:
- Personal care and hygiene.
- Nutrition and meal preparation.
- Household tasks and maintaining a clean living environment.
- Budgeting and financial management.
- Shopping and managing appointments.
- Medication routines (within organisational policies).
- Community access and use of public transport.
Community Participation
- Support residents to attend healthcare appointments.
- Encourage participation in education, employment, volunteering and meaningful activities.
- Promote social inclusion and healthy relationships.
- Support residents to access leisure activities and community resources.
Risk Management
- Follow individual risk assessments and positive behaviour support plans.
- Identify and report safeguarding concerns immediately.
- Promote least restrictive practice.
- Maintain professional boundaries at all times.
- Respond appropriately to emergencies and incidents.
Record Keeping
- Maintain accurate, contemporaneous and objective records.
- Complete daily notes, incident reports, body maps, observations and other documentation as required.
- Ensure all records comply with GDPR and organisational policies.
Health & Safety
- Follow infection prevention and control procedures.
- Administer or support with medication where trained and authorised.
- Report hazards, accidents and near misses.
- Participate in fire safety procedures and emergency evacuations.
Team Working
- Work collaboratively with colleagues and external professionals.
- Participate in handovers, supervision, team meetings and training.
- Contribute positively to service development and continuous improvement.
Professional Responsibilities
- Maintain confidentiality.
- Work in accordance with the Mental Capacity Act 2005, Mental Health Act 1983 (as amended), Human Rights Act 1998, Care Act 2014 and safeguarding legislation.
- Adhere to organisational policies, procedures and professional standards.
- Demonstrate the organisation’s values of compassion, respect, integrity, recovery and empowerment.
Pay: £13.00-£14.50 per hour
