We are looking for a forward-thinking, experienced Care Coordinator to join our team on an initial 9-month fixed-term contract to cover maternity leave, with the potential for the role to continue beyond this period depending on business needs and performance.

About the Role
As a Care Coordinator, you will play a key role in ensuring our care services are safe, effective, well organised and centred around the individual.
Your responsibilities will include:
- Coordinating and managing care rotas, ensuring appropriate staffing and continuity of care.
- Creating, reviewing and maintaining person-centred care plans.
- Completing initial assessments and supporting the development of care packages.
- Liaising with clients, families and representatives to understand their needs and ensure their wishes remain at the heart of their care.
- Working closely with health and social care professionals and other external agencies.
- Supporting the Field Care Supervisor and wider management team.
- Monitoring the delivery of care and responding proactively to any concerns or changes in client needs.
- Supporting compliance with company policies, procedures and relevant care standards.
- Participating in the on-call rota as required.
- Providing care and practical support where necessary to ensure continuity of service.
- Visiting clients in their homes and carrying out assessments, reviews and quality checks.
- Taking responsibility for identifying and resolving issues quickly and effectively.
- Contributing ideas and improvements to help KTG continue to develop and provide an excellent service.
About You
We are looking for someone who is proactive, accountable and genuinely client-focused.
You will ideally have:
- Previous experience working within the care sector.
- Experience in a care coordination, senior care or supervisory role is desirable.
- A good understanding of person-centred care and the needs of individuals receiving care at home.
- Confidence communicating with clients, families, care staff and health and social care professionals.
- Strong organisational and time-management skills.
- The ability to coordinate rotas and respond calmly to changing priorities.
- Excellent communication and interpersonal skills.
- The confidence to make decisions and take ownership of your responsibilities.
- A forward-thinking approach and a willingness to identify opportunities for improvement.
- The ability to work independently as well as part of a wider team.
- A full UK driving licence and access to a vehicle – essential, as the role involves travelling to clients' homes and completing initial assessments.
- Flexibility to participate in an on-call rota.
