Job Summary
We are seeking a dedicated and compassionate Care Coordinator to oversee and organise care services within a senior care environment. The successful candidate will be responsible for coordinating personalised care plans, supervising care staff, and ensuring the highest standards of service are maintained. Responsibilities
- Develop, implement, and regularly review personalised care plans tailored to individual residents’ needs.
- Coordinate daily care activities ensuring timely and appropriate service delivery.
- Supervise and support care staff, providing guidance and leadership to ensure high standards of care are maintained.
- Liaise effectively with residents, families, healthcare professionals, and external agencies to facilitate comprehensive care provision.
- Organise staff rotas and allocate resources efficiently to meet the needs of the care home.
- Monitor compliance with health and safety regulations, safeguarding policies, and quality standards.
- Assist with the training and development of team members to enhance their skills and knowledge.
- Utilise IT systems for documentation, scheduling, and communication purposes efficiently.
- Drive organisational initiatives aimed at improving resident satisfaction and operational efficiency.
Requirements
- Proven experience in senior care or a similar role within a care home environment.
- Supervising or leadership experience in a healthcare setting is essential.
- Strong organisational skills with the ability to manage multiple priorities effectively.
- Excellent communication skills, both written and verbal, with an empathetic approach towards residents and their families.
- Valid driving licence is preferred to facilitate transportation needs if required.
- Knowledge of care planning processes and relevant legislation governing adult social care.
- Proficiency in IT systems used for documentation and communication within healthcare settings.
- Ability to work collaboratively as part of a multidisciplinary team whilst demonstrating initiative when needed.
Pay: £14.00-£15.00 per hour
