We are looking for a Clinical Pharmacist to support a range of services within our Frailty Service.
Our Clinical Pharmacists work as part of a multidisciplinary team, alongside doctors, specialist nurses, pharmacy technicians and therapists, to deliver comprehensive geriatric assessments for patients in:
- the Jean Bishop Integrated Care Centre
- care homes
- community bedded units
- the Hospital at Home Frailty Virtual Ward, including home visits
We have vacancies available to support these services and projects. Usual working hours are Monday to Friday, 8.30am to 4.30pm. Part-time, flexible working and secondment arrangements will be considered.
Experience of structured medication reviews is desirable but not essential. Training will be provided through an individualised induction programme, and you will be supported by an experienced frailty team. CHCP also offers development and training opportunities tailored to your learning needs. The successful pharmacist will be patient-centred, interested in frailty, and have excellent communication and organisational skills. Applications are welcome from pharmacists from all sectors.
You will receive professional support from our established Medicines Service team in Hull. Access to independent transport is essential, as the role involves travel across the service area. You must also be able to work flexibly to meet organisational needs.
