About the Role
As a Mental Health Outreach Support Worker, you will provide one-to-one support to adults living with mental health needs in their own homes and within the community. You will encourage independence, promote recovery, and support people to achieve their personal goals.
No two days are the same. You will build positive relationships with the people you support while encouraging confidence, choice and independence.
Main Responsibilities
- Provide person-centred support in clients' homes.
- Support people with daily living activities and maintaining a safe home environment.
- Encourage independence and emotional wellbeing.
- Support clients to attend appointments and access community activities.
- Assist with shopping, meal preparation and light domestic tasks where required.
- Prompt or assist with medication in accordance with care plans and company policies.
- Accurately complete care records using our digital care system.
- Follow safeguarding procedures and report any concerns promptly.
- Work closely with families, healthcare professionals and the wider support team.
Essential Requirements
- Full UK Driving Licence.
- Own reliable vehicle with business insurance.
- Right to work in the UK.
- Caring, compassionate and reliable.
- Able to work independently and manage your own workload.
- Willing to work flexible hours, including some evenings and weekends if required.
- Experience working in domiciliary care or community outreach.
