Health and Wellbeing Practitioner – Kingfisher Leisure Centre
Part-time, Permanent
Salary £25,119 per annum pro-rata (£11,880 for 17.5 hours per week)
Active Living is a unique, award-winning Exercise Referral scheme designed to support people to increase their physical activity levels and improve their health. This initiative is a partnership between Abbeycroft, West Suffolk NHS Foundation Trust and Allied Health Professionals. Experience instructors will support patients through a programme of activity with follow-on options available for everyone to promote long term behaviour change. Our person-centred approach will ensure all aspects of health relevant to the client are considered and supported.
As a Health and Wellbeing Practitioner you will be required to coordinate the Active Living scheme services and support the delivery of required sessions/classes. You will support and empower the client to use their skills and networks to overcome barriers and make positive changes to improve their health and wellbeing. You will also support clients with more complex needs, which may include clients with various long term health conditions and/or issues with accessibility.
Your main area of responsibility will be to coordinate our Exercise on Referral scheme and relevant services, leading the management and where required lead on instruction and delivery. Carry out health and fitness assessments, consultations, and inductions including the provision of safe and effective exercise programmes. You will develop sustainable physical activity continuation to support lasting lifestyle changes and support and oversee where appropriate new projects to ensure the delivery of a varied and balanced portfolio of exercise and health initiatives. You are allocated time to ensure an excellent service is adhered to whilst achieving business, financial and operational targets by ensuring that the key performance indicators are met, evaluated and reported on in line with the organisational requirements.
Part-time: 17.5 hours per week
Working pattern: Tuesday 9.30-17.30 Thursday 9.30-17.30*
*This is a hybrid role working two days on site per week. The remaining contracted hours to be allocated to admin tasks worked remotely, day/time to be agreed with the successful candidate.

Key Responsibilities:
· Support the daily operations of the health pathways and related work
o Duties include providing support to both colleagues, clients, and health partners – collating data, triage clients, interacting with health professionals and clients to build good relationships
o Provide client triage, health assessments and inductions of clients accessing the Active Living service, including the provision of safe and effective exercise prescription
· Provide clear, consistent and professional communications with clients, partners, and colleagues to maintain and enhance service delivery
· Provide strong client liaison; with high levels of customer experience responding to general queries, problem solving, and ensuring quality of delivery
· Support with service delivery covering schemes when needed – including ensuring appointments/classes/sessions/services are covered where appropriate
· Support client continuation – supporting centre leads to create ongoing routes for clients / patients to continue using activities and services
· Complete re-assessments and evaluations of clients to monitor their progress, ensuring that appropriate data is collected and recorded and reporting on outcomes as required.
· Support data entry into the case management system ensuring accuracy and efficiency, whilst contributing to the measurement of change and improvement in health behaviours using relevant outcome measures
· Requirement to undertake any such other duties as may reasonably be required compatible with and/or arising from those listed above

· Identify and support clients with complex needs to access the most appropriate service and sustainable physical activity to support lasting lifestyle changes.
· Understand the wider determinants of health and barriers to healthy lifestyles and reduce health inequalities impacting on health behaviours.
· To undertake such other duties as may reasonably be required compatible with and/or arising from those listed above.

Requirements:
· Responsible individual with a passion for working in health and well-being.
· Strong understanding of wider health determinants and strengths and barriers faced by residents in your locality
· Experience of supporting people to access local services to support health and wellbeing
· Knowledge of behaviour change theories and techniques and experience of utilising these with clients to make healthy changes
· A strong understanding of referral, triage, and assessment processes
· Experience of managing a challenging workload through periods of high demand, and adapting to operational pressures
· Act as a local ambassador for healthy behaviour change
· Proven health coaching skills and ability to effect behaviour change
· Strong commitment to safety and the well-being of others.
· Ability to work well both independently and as part of a team.
Qualifications:
· Level 3 Fitness Qualification essential requirement
· Level 3 Exercise Referral Qualification required
· Relevant health coaching qualification or an accredited health coaching skills programme preferred
· First Aid at work qualification required
Candidates without a Level 3 fitness qualification will not be eligible for this role. First Aid at work certifications can be attained with Abbeycroft.
