Job Purpose
To support the design, delivery and evaluation of targeted health and wellbeing initiatives that improve physical, mental and social wellbeing and help individuals manage their health more effectively. The postholder will work with individuals and groups to promote healthier lifestyles, build confidence and self-management skills, and reduce the impact of health conditions and other barriers on people’s ability to access, remain in or progress towards employment.
- Support the design and delivery of targeted health and wellbeing programmes for individuals and groups.
- Provide one-to-one and group-based support to help participants improve their physical, mental and social wellbeing.
- Work with individuals to understand their health and wellbeing needs and identify appropriate goals, activities and support strategies.
- Encourage participants to make informed choices about their health, lifestyle and wellbeing.
- Support people with long-term health conditions or multiple barriers to employment to develop practical strategies for managing their health.
- Deliver engaging health and wellbeing activities, workshops and interventions tailored to the needs of different groups and individuals.
- Signpost and refer participants to appropriate health, wellbeing, community and specialist services where additional support is required.
- Build positive and supportive relationships with participants, encouraging engagement, confidence, motivation and sustainable behaviour change.
- Monitor participation, progress and outcomes, maintaining accurate records and completing relevant documentation.
- Support the evaluation of programmes and initiatives, gathering feedback and using information to identify opportunities for improvement.
- Work collaboratively with colleagues, health professionals, community organisations, employment services and other partners to provide coordinated support.
- Promote equality, inclusion and accessibility when designing and delivering health and wellbeing activities.
- Maintain appropriate confidentiality and comply with relevant safeguarding, health and safety, data protection and organisational requirements.
- Work flexibly across different locations and travel throughout the county to meet service and participant needs.
- Experience of supporting the delivery of health, wellbeing, lifestyle, community development, employment or related programmes.
- Understanding of the factors that influence physical, mental and social wellbeing.
- Experience working with individuals who may have long-term health conditions, complex needs or multiple barriers to employment is desirable.
- Ability to provide effective one-to-one and group-based support.
- Strong communication and interpersonal skills, with the ability to engage and motivate people from diverse backgrounds.
- Ability to build trusting relationships and encourage individuals to make positive and sustainable changes.
- Good organisational skills, with the ability to manage competing priorities and maintain accurate records.
- Ability to contribute to the monitoring and evaluation of programmes and use feedback to improve service delivery.
- Ability to work collaboratively with internal and external partners and make appropriate referrals or signposting.

Location: Carmarthen, Countywide, with flexible working and travel across the county as required.
Hours Per Week: 37 hours per week.
Bi-Weekly Payments: Paid fortnightly.
