We are currently looking for highly motivated dynamic individuals to join Torfaen Social Care and Housing Adult Services. Adult Services is a multi-disciplinary division that works together with partner agencies to meet the social care needs of older people, people with physical and mental health needs within Torfaen. Our aim is to work proactively with individuals, to promote choice, control, positive risk taking to achieve positive outcomes in areas that matters most to them.
The successful candidate will be a registered social worker who will be based in the Pontypool Wellbeing Team. They must be committed to work in a way which focuses on ‘what matters’ to the individual. We are looking for flexibility, enthusiasm and dedication to working positively with individuals in the community and hospital settings. You will need to be able to form and develop positive relationships with individuals and carers to ensure that they are supported to achieve outcomes that matter to them.
