Job Overview
We are seeking a dynamic and dedicated Senior Community Care Worker / Field Supervisor to lead and support community-based care services. Duties
- Supervise and support a team of community care workers, ensuring they deliver compassionate, person-centered care aligned with individual care plans
- Conduct regular visits to clients’ homes to assess needs, monitor service quality, and adjust care plans as necessary
- Lead the planning and scheduling of care visits, ensuring timely and efficient service delivery across your designated area
- Provide coaching, training, and mentorship to care staff to enhance their skills and ensure compliance with health and safety standards
- Maintain accurate documentation of client interactions, care activities, and staff records using IT systems
- Communicate effectively with clients, families, healthcare professionals, and team members to coordinate care and address any concerns promptly
- Drive compliance with all regulatory requirements, safeguarding policies, and company procedures while promoting best practices in community care
- Carry out new client assessments, Conduct risk assessment, Spot checks, Staff supervisions, Competency assessments, Medication audits, Support new carers, Participate in the on-call rota, Liaise with hospitals and social workers, Quality assurance visits
Qualifications
- Proven experience working in senior or community care environments, ideally within a care home, domiciliary care or similar setting
- Supervising or leadership experience in a healthcare or social services context is essential
- Valid driving license and ability to travel between client locations as required
- Strong understanding of care plans, safeguarding procedures, and health & safety regulations
- Excellent communication skills with the ability to motivate and lead a diverse team
- Proficiency in using IT systems for documentation and reporting purposes
- Knowledge of senior care needs and best practices for delivering respectful, person-centered support

Pay: £16.50-£19.00 per hour
