We are pleased to be advertising for a Band 4 Associate Practitioner to work with us in our Enhanced Recovery and Support at Home Team. You will be working as part of a team consisting of Physiotherapy, Occupational Therapy, Nurses and Rehab Support workers assistants.
The service operates 8am-6pm 7 days a week and you will be expected to work on a rota basis to cover these hours.
The team has a clinical supervision structure and there is access to comprehensive in-service training.
With a particular focus on rehab, falls and provision of equipment for patients at home, you will have experience in working autonomously as delegated by the OTs and Physios, be able to make your own assessments of patients’ risks and needs, and undertake rehab treatment programmes.
You will receive direct line management from a registered therapist who will support you to develop within the role , both in terms of confidence as well as competence, and access further training and development opportunities to help you expand your skills and knowledge as appropriate .

As this is a community post, you must have access to a vehicle for work purposes with business insurance and, although generally rostered to work Monday to Friday, some weekend and evening shifts may be required.
Deliver high quality care support and assessments in people's homes, under guidance of a registered therapist
Undertake initial assessments of new patients as delegated by registered therapists, including early identification of risks and development of appropriate care plans , while promoting patient independence;
Provide support to the community therapies teams and community rehab wards as needed
Be responsible for own caseload, including assessment, rehab progression and discharge planning as delegated by registered therapist
Demonstrate capacity to identify, review and risk assess basic equipment needs and issue them in the community autonomously
Be able to travel in between patients addresses in a timely manner and organise visits efficiently
To work within relevant legal and ethical frameworks and in accordance with organisational protocols and policies.

Provide patient focused care, support, education rehabilitation to patients to support the delivery of an agreed plan of care. Promote patient independence. Support the patients to achieve their individualised goals

Provide the range of clinical support as required within Intermediate Care and the community rehab wards
Following training, be responsible for a clinical case load in consultation with the relevant professionals and following codes of practice and professional guidelines
Demonstrate safe and effective moving and handling and positioning skills specific to patients’ care. This may include use of specialist equipment
Be enrolled on Associate Practitioner course (or equivalent) within one year
Participate in team projects, including audit and quality initiatives as requested
The post involves driving, lone working and the possibility of exposure to unpleasant working conditions in patients’ own homes. To have business insurance on your vehicle.
