Caafi Health CIC is a Bristol based organisation focusing on minoritised communities to reduce health inequalities. Working in partnership with Macmillan Cancer Support we are launching Walking With You, a two year project tackling cancer care inequalities across Bristol, North Somerset & South Gloucestershire (BNSSG).
We are looking for an experienced registered clinician to be a trusted link between people living with cancer and the NHS services caring for them. You will support people through treatment decisions, spot opportunities to strengthen cancer pathways, and work with NHS colleagues to make those improvements happen.
Funded clinical supervision, a training budget, hybrid and flexible working, 25 days leave plus bank holidays pro rata, and a workplace pension.
Responsibilites
- Provide one to one clinical navigation and advocacy for people living with cancer
- Support people to prepare for appointments and take part in shared decision making
- Coordinate referrals and continuity of care across trusts, GP practices and community services
- Lead pathway mapping and escalate systemic failures through the steering group and NHS forums
- Co-design and deliver learning sessions for health and care staff
- Maintain records on our CRM and contribute to quarterly monitoring and evaluation
Requirements
Essential
- Current UK clinical registration (NMC, HCPC, GMC, GPhC or equivalent)
- Substantial post registration experience across cancer or long term condition pathways
- Experience of advocacy, navigation or care coordination in complex health systems
- Confident influencing senior clinical and system colleagues across organisational boundaries
- Strong understanding of the barriers Black and racially minoritised communities face in cancer care
- Able to travel across BNSSG. Enhanced DBS clearance required, which we will arrange
Desirable
- Experience in cancer services, oncology or palliative care
- Experience of quality improvement, co-production or delivering staff training
- Ability to speak a community language such as Somali, Arabic, Urdu, Bengali or Tigrinya
