- Contract - Full time, Fixed term until 30/07/2027
- Hours - 37.5 hours per week
- DBS requirement - Enhanced

This role offers a starting salary within Nacro’s pay structure of £31,982 with expected progression to £32,782 within the first year, subject to meeting clear criteria and performance standards, with further pay progression available in the future. Help people build lasting recovery, as Residential and Aftercare Coordinator, you will coordinate and manage the complete rehabilitation pathway for individuals accessing residential treatment services.
You'll work closely with clients, rehabilitation providers, healthcare professionals, housing services, and community organisations to ensure each individual receives tailored support that meets their needs and promotes long-term recovery.
This is a varied role combining assessment, case management, partnership working, budget oversight, discharge planning, and aftercare coordination.
You will bring with you:
- Knowledge of substance misuse treatment pathways, rehabilitation services, and recovery-focused interventions.
- Experience supporting individuals with complex needs and managing caseloads.
- Strong assessment, care planning, and case management skills.
- Excellent communication and relationship-building abilities.
- The ability to work effectively with multiple agencies and stakeholders.
- Strong organisational skills and the ability to manage competing priorities.
- Experience maintaining accurate records and monitoring outcomes.
- A commitment to safeguarding, confidentiality, and person-centred practice.
