Health Captains Programme Lead
Salary: £30,000 pa
Hours: 35 hours per week (full time)
Location: The Strand, 75 Beverley Road, Hull.
Contract: Fixed term until Spring 2027 – with the potential for extension subject to further funding
The Health Captains programme is an innovative service which aims to reduce health inequalities and reduce pressures on NHS services.
The aim of the Health Captains programme is to identify existing volunteering programmes across the Humber area which support individuals either in accessing health care services or in managing their recovery and condition; to identify further opportunities whereby volunteers could support individuals in this way; and working with healthcare and voluntary sector partners to develop and implement new such initiatives.
A key deliverable for this year is to support the VCSE Discharge Collaborative; a partnership of VCSE organisations that provide practical targeted support to patients post discharge from hospital to help prevent readmission.
This is a fast paced and sometimes emotionally challenging role, yet also extremely rewarding. You will get to know patients who have recently left hospital; help identify challenges they are facing with their long term condition; and provide practical, non-clinal advice. You will be supported by a team of volunteers with lived experience of those long term conditions, to help make a real difference to patients.
We are looking for someone who understands the health and care arena and who can form relationships quickly and attract interest and involvement from NHS and voluntary sector partners and volunteers.
This is a multifaceted role and will include recruiting and supervising volunteers, coordinating referrals, speaking to and listening to patients around their needs, liaising with partners in health and social care and the wider community to help address identified needs, and recording and reporting on activity delivered.
Whilst the focus of the programme this year will be on the VCSE Collaborative, there is much scope to develop the programme beyond this initiative. Therefore we are looking for someone who can also spot new opportunities to develop the service further, that will provide additional ways to address inequalities, and help people move through care services more quickly.
