We are looking for an experienced SEMH Support Worker / Transition Specialist to support a secondary-aged young person within our trauma informed, non-contact Alternative Provision in Calderdale.
This is a 2:1 role focused on helping a young person build the confidence, regulation, routines and practical skills needed to move gradually towards small-group learning and a future school placement.
The role combines pastoral support, practical learning and carefully planned transition work.
The purpose of the role
You will work as part of a two-person team providing consistent, relationship-based support.
A key part of the role is helping the young person move from individual support into appropriate small-group practical workshops when they are ready.
When group sessions are not accessible, you will help deliver an individual programme of practical and hands-on learning, supported by senior management and linked where appropriate to AQA Unit Award Scheme modules.
The aim is not simply to supervise the young person, but to help them make measurable progress towards engagement, confidence, independence and a suitable future education placement.
Key responsibilities
- Provide consistent 2:1 support
- Build a safe, appropriate and trusting professional relationship
- Hold daily pastoral check-ins
- Support emotional regulation and engagement
- Help identify barriers to learning and participation
- Support gradual integration into small-group practical workshops
- Deliver individual practical learning activities when group sessions are not accessible
- Support learning linked to AQA Unit Award Scheme modules
- Help break transition goals into clear, manageable steps
- Encourage independence and confidence
- Maintain clear professional boundaries
- Work closely with senior management and other staff
- Contribute to monitoring progress and adapting support where needed
About you
We are particularly interested in applicants who:
- Have direct experience supporting children or young people with SEND and/or SEMH needs
- Understand how anxiety, regulation and previous educational experiences can affect engagement
- Can remain calm and consistent when a young person is struggling
- Can build positive relationships while maintaining professional boundaries
- Are confident supporting practical, hands-on learning
- Can adapt activities and expectations to meet individual needs
- Can support a young person to move gradually towards greater independence
- Work well as part of a small team
- Understand safeguarding responsibilities
- Believe practical learning can be a powerful route back into education
Experience in Alternative Provision, pastoral support, behaviour support, youth work, SEND education or specialist support would all be relevant.
Pay: £17.35 per hour
