About the Role
As a Care Coordinator, you will:
- Coordinate each shift to ensure residents receive safe, person-centred care
- Administer medication safely and accurately in line with residents’ care plans, prescriptions and the home’s medication policies
- Complete medication stock checks, audits and accurate MAR documentation
- Lead, support and provide guidance to care staff, allocating duties appropriately
- Review and follow care plans, risk assessments and associated documentation
- Support residents with meals and hydration, ensuring their nutritional needs are met
- Monitor residents’ health, promptly reporting and responding to any changes
- Complete effective handovers and ensure important information is communicated
- Escalate safeguarding, clinical or operational concerns appropriately
- Ensure staff follow the home’s policies, procedures and expected standards of care
- Promote residents’ dignity, choice, privacy and wellbeing at all timesAbout You
We are looking for someone who:
- Has previous experience as a Care Coordinator, Team Leader or Senior Care Assistant
- Is confident in leading, supporting and motivating a care team
- Remains calm, organised and professional when working under pressure
- Has excellent communication skills and can work effectively with residents, relatives, colleagues and healthcare professionals
- Is reliable, approachable and able to manage competing priorities
- Has a genuinely caring nature and promotes dignity, respect, choice and independence
