Purpose of the Role
As a Positive Behaviour Support (PBS) Support Worker at Together Livin, you will provide high-quality, person-centred support to individuals with learning disabilities, autism, complex needs, and behaviours of concern. You will work alongside the wider support team to implement Positive Behaviour Support plans, promote independence, improve quality of life, and reduce behaviours of concern through proactive, compassionate, and evidence-based approaches. You will support individuals to achieve positive outcomes while ensuring their safety, dignity, and wellbeing.
Key Responsibilities
- Deliver person-centred support in line with Positive Behaviour Support plans, risk assessments, and individual care plans.
- Build positive relationships with individuals, promoting choice, independence, communication, and community inclusion.
- Use proactive and preventative strategies to reduce behaviours of concern and support positive outcomes.
- Respond calmly and appropriately to behaviours of concern, using de-escalation techniques and least restrictive approaches.
- Assist with personal care, medication administration, meal preparation, and daily living activities where required.
- Accurately record behavioural observations, daily notes, incidents, and progress to support ongoing reviews of PBS plans.
- Work collaboratively with families, healthcare professionals, and multidisciplinary teams to ensure consistent support.
- Participate in team meetings, training, reflective practice, and supervision to develop knowledge and skills in Positive Behaviour Support.
- Follow safeguarding, health and safety, confidentiality, and data protection policies at all times.
