Winchester City PCN is looking for a compassionate and motivated Proactive Care HealthProfessional to support patients living with frailty, multiple long-term conditions and complex health needs.
This rewarding role focuses on identifying patients at risk of deterioration, providing holistic assessments, coordinating personalised care, and helping prevent avoidable hospital admissions.
Working across our multidisciplinary teams, you'll collaborate with GPs, pharmacists, community services, social care and voluntary organisations to deliver truly joined-up care.
What You'll Do
- Manage a caseload of frail and complex patients.
- Carry out comprehensive frailty and holistic assessments.
- Develop personalised care and support plans.
- Identify and respond to early signs of deterioration.
- Support medication reviews alongside PCN pharmacists.
- Coordinate care across health and social care services.
- Facilitate advance care planning conversations.
- Promote independence, self-management and wellbeing.
What We're Looking For
- Registered Nurse with current NMC registration or a UK professional with a relevant health degree
- Experience working with older people, frailty or complex long-term conditions.
- Strong communication and relationship-building skills.
- Experience working within multidisciplinary teams.
- Ability to work independently and manage a varied caseload.
- Full UK driving licence and willingness to travel across the locality.
