Our highly skilled Community Nursing teams assess, plan and deliver evidence-based nursing care, helping patients manage their health and wellbeing in their own community.
As a Community Staff Nurse, you will provide high-quality, holistic nursing care to patients within their own home environments or within our ambulatory clinics. You will work alongside our experienced, supportive and friendly colleagues, and will develop your skills in a dynamic environment while delivering person-centred care to individuals with increasingly complex health needs.

As a highly skilled NMC Registered nurse you will work in collaboration with the wider muti-disciplinary team and colleagues to:
Assess patients’ needs.
Promote independence and support patient centred care planning.
Provide care to patients on the caseload with a range of nursing needs (including: palliative & end of life care, long term condition management, medication administration, complex wound care and bladder & bowel management)
You will work autonomously, in line with your scope of practice, competence and policies.
Handover & document patient information effectively.
Escalation of patients’ needs as appropriate.
Lone working with the support from the wider team; traveling independently over the geographical patch.

Working pattern
These roles involve daytime working, including weekends, with occasional early morning shifts starting from 7:00am where required.
Travel requirements
Due to the nature of community nursing, you will need to be able to travel across the locality to provide care.
