Are you ready to make a real impact? Join our Mental Health Discharge Team as a Social Worker in Adult Social Care. This fast-paced role focuses on supporting individuals in acute psychiatric wards, ensuring timely and person-centred discharge planning. You will work closely with health and social care partners to help people transition smoothly from hospital to home.
As a Social Worker, your key responsibilities will include:

Conducting strengths-based assessments and discharge planning.

Collaborating with individuals, families, carers, and partner organisations to understand what matters most.

Commissioning care and support to enable safe and timely discharges.

Supporting individuals to live independently, focusing on reablement and community-based support.

Contributing to improved outcomes for people with mental health needs.
To succeed in this role, you will need:

A recognised Social Work qualification.

Registration (or eligibility to register) with Social Work England.

Knowledge of the Mental Health Act and experience working with mental health.

Understanding of community care services and relevant legislation in adult social care.
