We are excited to announce an opportunity for an Experienced Social Worker to join our Transforming Lives Team within the Community Forensic Team (CFT). In this vital role, you will support individuals with complex needs, ensuring they receive the care and supervision they require. Your expertise in the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983 will be crucial as you undertake assessments, support planning, and risk assessments for forensic patients. As a Social Worker in our Community Forensic Team, your key responsibilities will include:

Conducting Care Act assessments and developing tailored support plans.

Performing risk assessments and managing the discharge of forensic patients.

Providing social supervision and reporting to the Ministry of Justice for individuals under section 41 of the Mental Health Act.

Collaborating with a multi-disciplinary team to ensure comprehensive care and management.

Supporting individuals and families with complex needs, promoting their independence and wellbeing.
To excel in this role, you will need:

A Social Work Qualification recognised by Social Work England.

Registration with Social Work England (or equivalent).

A minimum of 2 years’ post-qualification experience in social work, particularly in complex situations.

Knowledge of community care services and relevant legislation.

Experience in safeguarding investigations and managing caseloads independently.
