Purpose of role:
To support delivery of Community Transformers’ charitable programmes by engaging beneficiaries, assessing community needs, coordinating practical support, and helping vulnerable individuals access appropriate services.
Specific job description:
The postholder will work directly with service users, volunteers, partner organisations and local stakeholders to deliver community-based support programmes. Duties may include identifying beneficiary needs, signposting individuals to relevant services, supporting outreach activities, maintaining case notes, assisting with safeguarding procedures, coordinating community events, and helping deliver projects that improve wellbeing, inclusion and access to support.
Key duties:
- Engage with individuals and families who need community, welfare, employability, wellbeing or social support.
- Carry out initial needs assessments and refer beneficiaries to appropriate internal or external services.
- Support the planning and delivery of outreach sessions, workshops, community events and support groups.
- Maintain accurate beneficiary records, attendance logs and monitoring information.
- Work with volunteers and partner agencies to ensure beneficiaries receive timely and appropriate assistance.
- Promote safeguarding, equality, confidentiality and dignity in all service delivery.
- Provide feedback to management on emerging community needs and service gaps.

· Good organisational skills, including the ability to manage appointments, outreach sessions, records and follow-up actions.
· Ability to work independently in the community and collaboratively as part of a team.
· Problem-solving skills and the ability to respond calmly to challenging or sensitive situations.
· Basic IT and record-keeping skills, including use of Microsoft Office, email, spreadsheets and beneficiary databases.
· Awareness of local support services, including welfare, housing, employability, health, wellbeing, food support and advice agencies.
Experience required
· Previous paid or voluntary experience in community work, charity work, outreach, support work, advice services, youth work, social care, employability support, health and wellbeing projects or a similar frontline role.
· Experience of engaging people from diverse backgrounds and supporting people facing barriers such as poverty, isolation, unemployment, health needs, language barriers or social exclusion.
· Experience of assisting with community events, workshops, drop-ins, support groups or outreach activities.
· Experience of maintaining accurate case notes, attendance logs, referral records or monitoring information.
· Experience of working with volunteers, community groups, local authorities, faith groups, charities or statutory agencies would be beneficial.
· Experience of following safeguarding, confidentiality and data protection procedures in a service delivery setting.
Qualifications and training required
· A first degree is desirable for record-keeping, communication and report writing.
· A Level 2 or Level 3 qualification in community development, health and social care, advice and guidance, youth work, social care, counselling skills or a related field would be advantageous.
· Safeguarding adults and children training should be completed before or shortly after appointment.
· Training in equality, diversity and inclusion, confidentiality, data protection and professional boundaries is recommended.
· First aid, mental health first aid, trauma-informed practice, domestic abuse awareness or conflict de-escalation training would be beneficial depending on service user needs.
· An enhanced DBS check may be required where the role involves regulated activity or regular work with vulnerable groups.
· A driving licence may be desirable if outreach work requires travel across community locations.
Pay: £26,000.00-£34,000.00 per year
