Overview
We are seeking a dedicated and compassionate Home Care Nurse to provide high-quality nursing care to patients in their own homes. Responsibilities
- Deliver comprehensive nursing care tailored to individual patient needs within their home environment
- Develop, implement, and regularly review personalised care plans in collaboration with patients and their families
- Monitor patient health status, administer medication, and perform routine health assessments
- Provide education and support to patients and carers regarding health management and treatment procedures
- Coordinate with multidisciplinary teams to ensure continuity of care and optimise patient outcomes
- Maintain accurate documentation of all care activities, observations, and interventions in accordance with regulatory standards
- Lead by example in maintaining high standards of hygiene, safety, and infection control practices
- Offer leadership and guidance to junior staff or carers involved in patient care where necessary
Requirements
- Nursing qualification or relevant caregiving experience.
- Good communication skills in English.
- Kind, patient, and trustworthy.
- Willing to work as a live-in caregiver.
