Clarence Healthcare Services is a CQC-registered domiciliary care provider delivering person-centred care and support to adults in their own homes. We are expanding our services in Hull and are seeking an experienced, organised and dependable Care Coordinator to support the safe mobilisation and ongoing coordination of care packages.
The Role
The Care Coordinator will work closely with the Registered Manager and Field Supervisor to coordinate care delivery, support care workers and help maintain high standards of quality and compliance.
This is a flexible zero-hours position suited to someone with previous domiciliary care experience who can work according to service demand.
Key Responsibilities
- Prepare and maintain staff rotas using an electronic care management system.
- Match care workers to service users based on skills, availability, location and individual preferences.
- Coordinate new care packages and support safe service mobilisation.
- Monitor late, missed or shortened calls and take prompt corrective action.
- Maintain accurate electronic care records and scheduling information.
- Liaise with service users, relatives, social workers, commissioners and healthcare professionals.
- Support staff induction, shadowing and ongoing development.
- Carry out announced and unannounced spot checks in service users’ homes.
- Complete care-worker observations and report any competency or performance concerns.
- Support care-plan reviews, risk-assessment updates and service-user welfare checks.
- Monitor staff training, DBS, right-to-work and compliance documentation.
- Escalate safeguarding, medication, health and safety or quality concerns promptly.
- Assist with complaints, incidents and service improvement actions.
- Provide emergency rota cover and participate in the on-call rota where agreed.
- Promote dignity, independence, choice and person-centred care.
- Support compliance with CQC standards, company policies and local authority contract requirements.
Essential Requirements
- Previous experience in domiciliary care, home care or community support.
- Experience coordinating care rotas or supervising care staff.
- Good understanding of person-centred care, safeguarding and confidentiality.
- Strong organisational and communication skills.
- Ability to respond calmly to staffing changes and urgent situations.
- Competence using email, Microsoft Office and electronic care systems.
- Ability to travel across Hull and surrounding areas.
- Right to work in the UK.
Desirable Requirements
- Level 3 Diploma or NVQ in Health and Social Care.
- Experience completing spot checks, supervision or competency observations.
- Knowledge of CQC Fundamental Standards.
- Experience working with council-funded care packages.
- Experience using electronic rostering, eMAR or care-planning systems.
- Medication, moving and handling, safeguarding and mental-health training.
- Full UK driving licence and access to a vehicle.
Pay: £14.00-£15.00 per hour
