About the role:
At Recovery Connections, we believe that people can and do recover. As our Admission and Discharge Coordinator, you’ll play a vital role in supporting people at some of the most important transition points in their recovery journey.
You’ll be the link between referral, admission, and successful completion—helping people access residential rehabilitation and move forward with confidence, dignity, and the right ongoing support.
Working across our CQC-rated Outstanding rehab provision, you’ll collaborate with a wide network of partners and ensure every individual receives a safe, supportive, and well-coordinated experience.

Qualifications
- Level 2 (or above) in a relevant Health & Social Care subject
- (Desirable)
- Coaching, teaching, or professional qualification in a related fieldWhat You’ll Be Doing
Support people through their recovery journey
- Guide and coach individuals through admission and discharge
- Carry out assessments and help determine readiness for rehab
- Provide encouragement, motivation, and practical support throughout
Coordinate admissions and discharges
- Manage the end-to-end process across residential rehab services
- Support both planned and unplanned discharges
- Ensure continuity of care and smooth transitions into community support
Work collaboratively
- Partner with services across mental health, criminal justice, and healthcare
- Attend and contribute to multi-disciplinary meetings
- Act as a key contact for partner agencies
Maintain quality and compliance
- Keep accurate records, care plans, and outcome measures (e.g. NDTMS, TOPs)
- Ensure all work meets CQC and organisational standards
- Contribute to reporting, reviews, and service improvement
Support safe, person-centred environments
- Complete risk assessments and support risk management planning
- Maintain safe, welcoming environments for residents
- Support inductions, including orientation to accommodation and policies
Promote recovery and independence
- Support access to community resources, housing, and peer support
- Encourage development of recovery capital
- Deliver harm minimisation and brief interventions where needed
About You

Experience of:
- Working in the recovery/addiction or health and social care sector
- Carrying out assessments and supporting individuals directly
- Working with multiple agencies and stakeholders
