This is a senior, advanced-level clinical position providing expert clinical leadership across our 62-bedded hospital. The post-holder will operate with a high degree of autonomy, undertaking comprehensive health assessments, formulating differential diagnoses, initiating and reviewing treatment plans, and independently prescribing medications. A defining feature of this role is the integration of physical and mental health care — ensuring parity of esteem between both — and driving measurable improvements in the physical health outcomes of individuals living with severe mental illness.
2. Job Purpose
Operating at an advanced level of clinical practice, the post-holder will utilise expert knowledge, complex decision-making skills, and advanced diagnostic and therapeutic capabilities to deliver holistic, person-centred care within our 62-bedded independent mental health hospital at Seren Gobaith.
The primary focus of this role is to take an active, hands-on clinical lead in the physical health care of our patients, directly meeting the statutory requirements of the Independent Health Care (Wales) Regulations 2011 and the National Minimum Standards (NMS) for Independent Health Care Services in Wales, as enforced by Healthcare Inspectorate Wales (HIW). You will bridge the gap between mental and physical health care, systematically managing complex physical co-morbidities, addressing the adverse metabolic side-effects of psychotropic treatments, and ensuring that physical health care at Seren Gobaith enjoys true parity of esteem with psychiatric treatment.
Accountable directly to the Hospital Manager (Registered Manager under HIW), you will work in close collaboration with the Astons pharmacy team on medicines optimisation and reconciliation, lead a rolling programme of clinical audits, and ensure rigorous HIW regulatory compliance, operational efficiency, and clinical excellence across the Seren Gobaith site.

Qualifications & Registration
- Registered Nurse — Mental Health (RNMH/RNLD) or Adult (RN) with current NMC registration and no outstanding fitness-to-practise conditions. MSc in Advanced Clinical Practice (or equivalent postgraduate qualification at Level 7). Independent Nurse Prescriber (V300) qualification with evidence of current prescribing practice. Evidence of ongoing continuing professional development (CPD).
- Leadership, quality improvement, or teaching qualification. Advanced Life Support (ALS) or Immediate Life Support (ILS) certification. Recognised qualification in physical health assessment skills.
Experience
- Significant post-registration experience (normally 5+ years) in mental health, complex care, or psychiatric liaison settings. Demonstrable, recent experience of independently conducting advanced physical health assessments and diagnostic reasoning in a psychiatric or complex-care environment. Proven experience leading clinical audits, root-cause analyses, serious incident investigations, and quality improvement initiatives. Experience of supervising, mentoring, and/or assessing junior clinical staff or students. Experience of working within multi-disciplinary teams, including partnership working with pharmacy, medical, and therapy colleagues.
- Experience working within an independent (private) hospital setting regulated by HIW or an equivalent regulatory body. Experience managing physical health pathways for patients detained under sections of the Mental Health Act 1983. Experience of preparing for and participating in HIW/CQC inspections. Experience of working with the Astons pharmacy team or a similar clinical pharmacy provider.
Skills & Knowledge
- Expert knowledge of the interaction between psychotropic medications and physical health (metabolic, cardiac, endocrine, neurological side-effects). Thorough applied understanding of the Independent Health Care (Wales) Regulations 2011 and HIW National Minimum Standards. Expert knowledge of the Mental Health (Wales) Measure 2010, Mental Health Act 1983, Mental Capacity Act 2005, and Social Services and Well-being (Wales) Act 2014 as they apply to clinical practice. Advanced clinical diagnostic, clinical decision-making, and independent prescribing skills. Competence in history-taking, physical examination, clinical reasoning, and treatment planning. 6. Terms & Conditions of Employment

Salary: Competitive, commensurate with experience and qualifications.
Hours of Work: Full-time (40 hours per week).
Annual Leave: 5.6 weeks per year calculated pro rata to your contracted hours in accordance to the working time regulations 1998.
Notice Period: Within the first month -1 weeks’ notice. After the first month but before end of the third month -2 weeks. From the third month to the end of probation -3 weeks’ notice after successful probation period it will be 4 weeks’ notice.
Probationary Period: Six months, subject to review.
