Detailed job description and main responsibilities
The post holder will be responsible for managing their own workload, prioritising tasks effectively, and ensuring deadlines are met. They will be expected to work independently as well as part of a team, demonstrating flexibility and a commitment to high standards. The role involves working across two sites and requires excellent communication and organisational skills.
The successful candidate will be responsible for the daily management of patients on the Cancer PTL using Somerset the Trusts' Cancer Management System ensuring 28FDS and 62-Day referral to treatment standards are met including ensuring all relevant data is added efficiently and accurately contributing to mandatory COSD and CWT submissions for national reporting. To identify any delays in patient pathways and to escalate these appropriately to all stakeholders in line with National Cancer Wait Time Standards.
Cross checking all suspected cancer referrals against investigations and other diagnostic /reporting systems (e.g. Alex Health, Somerset, Solus, Medisight, Oncoalert ) to ensure all cancer patients are identified and inform MDT lead /nurse of newly diagnosed cancers to be discussed at MDT meeting (in line with Improving Outcome Guidance).
To deliver best service delivery for patients with Cancer, through the support of an efficient MDT decision process, in particular the coordination of MDT Meetings. Collect and provide good clinical and care pathway data and information management for use by MDT's and to meet Trust audit requirements. Working with the wider Cancer MDT Team to share tumour site / MDT workloads and support with specific tasks as required to meet demands. Cross cover of MDT meetings including those held out of hours if and when required.
To liaise with primary care providing updates and sharing information as appropriate regarding patients care and cancer management throughout their pathway including diagnostics, outcomes and treatment plans. To be able to communicate effectively and promote effective communication and relationships across the multidisciplinary groups.
