Reablement Team provides a short-term service designed to help people develop the confidence and skills they need to live as independently as they can.

Your next role
The Reablement Support Worker role is part of an integrated Reablement Team and the wider Adult Health & Social Care multidisciplinary teams, to provide support with personal care and reablement to service users in their own homes and community through outcome focussed support.
You will support service users by assessing service users’ needs and monitor against aims, objectives and outcomes as part of their individual care plan, and liaise with other professionals.
In addition, you may be asked to assist service users with bills, shopping and prescriptions and contribute to the promotion of positive links with local community agencies/groups. For very vulnerable service users, you will assist them in developing, regaining and maintaining skills to maximise their independence, this will include very personal care tasks, whilst always treating them with dignity and respect.
You will maintain accurate and up to date records and report any accidents or concerns.
The successful candidate must be highly motivated and enthusiastic about providing a first-class service to the residents of Swindon. Be able to work independently, using initiative and as part of the wider Reablement Team.
You will need to have experience and knowledge of caring for vulnerable adults with a wide range of complex needs, including personal care and have experience of working in the community or care home.
The ability to travel throughout the Borough is essential and you will need a full driving licence and access to a vehicle whilst working.
You will be subject to an enhanced Disclosure and Barring Service Check
You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act “for the effective performance of a customer-facing role”.
