About the Role
Are you passionate about supporting people in your community? We are looking for a caring, motivated, and compassionate Band 5 Community Worker to join our team across the East of England. You will play a key role in helping individuals maintain their independence, improve their wellbeing, and access the support they need within their own homes and communities.
What You'll Do
- Support service users with their health, wellbeing, and social care needs
- Carry out assessments and contribute to person-centred care plans
- Work closely with healthcare professionals, families, and partner agencies
- Promote independence and positive health outcomes
- Keep accurate and timely records
- Follow safeguarding, confidentiality, and professional standards
- Participate in team meetings, supervision, and training
What We're Looking For Essential
- Minimum 6 months of NHS or community healthcare experience
- Experience working with vulnerable adults, families, children, or community services
- Good organisational and time management skills
- Understanding of safeguarding and confidentiality
- Full UK driving licence and access to a vehicle
