Guardian Angel Care Bournemouth is growing, and we are looking for an experienced and organised Care Coordinator to join our friendly branch team.
This is a newly created position within the branch and an exciting opportunity to join us at an important stage of our growth. The branch is doing incredibly well, with new clients joining us and our team of carers continuing to expand.
We are looking for someone who wants to become an important part of the branch, support its continued development and help us make sure our clients receive reliable, consistent and high-quality care.
This is a busy and varied role. You will work closely with the Branch Manager and take responsibility for coordinating the care delivered across the Bournemouth and Christchurch areas.
Care coordination is not simply about filling spaces on a rota. It is about knowing your carers, understanding your clients and making sure the right person is in the right place at the right time.
The role will include:
- Creating, maintaining and managing the weekly care rota.
- Making sure all client visits are safely and appropriately covered.
- Matching carers to clients based on their needs, personalities, skills, availability and location.
- Responding to changes including sickness, holidays, emergencies and changes in client needs.
- Communicating regularly with carers, clients and their families.
- Supporting carers with day-to-day questions and concerns.
- Monitoring late visits, missed visits and changes to planned care.
- Helping to organise and introduce new packages of care.
- Supporting the Branch Manager with new client enquiries and assessments.
- Keeping client and carer records accurate and up to date.
- Working closely with recruitment to identify staffing requirements.
- Supporting new carers as they join the branch and begin working with their clients.
- Helping the branch continue to grow while maintaining excellent standards of care.
- Taking part in the branch on-call rota, for which an additional payment will be made.
- Supporting essential care visits in exceptional circumstances where cover cannot otherwise be found.
About you
You will need to be organised, confident and able to remain calm when plans change.
You must be someone who takes responsibility, communicates clearly and follows things through. You will need to be comfortable managing several priorities at once and dealing confidently with carers, clients and families.
Ideally, you will have:
- Previous experience working within domiciliary care.
- Experience in care coordination, rota planning, field care supervision or a similar role.
- A good understanding of person-centred care.
- Strong organisational and problem-solving skills.
- Excellent communication skills.
- Good attention to detail.
- Confidence using electronic care planning and rota systems.
- The ability to work independently while also being part of a close team.
- A full UK driving licence and access to a vehicle.
- Flexibility to take part in the on-call rota.
An NVQ Level 3 in Health and Social Care would be an advantage, although experience, attitude and a strong understanding of care are equally important.
