To work as part of a multi-disciplinary team, delivering health and social care, focusing on the crisis intervention of people experiencing mental health problems. Working autonomously with an allocated caseload, under supervision of a qualified clinician.
We are looking to appoint a team-player who is motivated and enthusiastic, committed to the delivery of the highest quality and excellence in mental health care.
Promoting good physical health is a key area in mental health, and our Assistant Practitioners play in a vital role in supporting the team, including the facilitation of physical health assessments and healthy lifestyles work in the community setting.
- Assess, plan, organise and deliver care as agreed with supervising clinician, in line with clinical governance
- Undertake and record Physical Health checks particularly during clozapine titration and support the physical well-being of people under our care
- Lead on the support for carers in the team ensuring that all carers are identified and supported whilst under the care of the crisis and home treatment
- To hold an identified caseload, under supervision
- To work autonomously, with the ability to reflect upon clinical with the supervising clinician/s
- Recognise and respond appropriately to urgent and emergency situations
- Document and maintain accurate clinical records within the trust’s electronic records system. To support the qualified clinicians in completing key aspects of data entry on Systemone.
- To provide effective evidence based care for people who use services and their families and carers under the supervision of qualified clinicians.
- To identify and report any concerns in regards to the potential abuse/neglect of people and carers in line with the Domestic Abuse, Surrey Safeguarding Adults and Children Processes.
- To participate in team audits and teaching sessions in the team as required
- To undertake any other duties at the request of the supervisor which are commensurate with the role.
