Job summary
We are seeking applications from Nurses or other registered Healthcare Professionals to work with us in North Hampshire supporting patients with Multiple Sclerosis (MS). Are you enthusiastic, with excellent interpersonal skills and a genuine commitment to improving the lives of people living with MS?
A keen interest in MS supported by experience of working with patients with MS/neurological conditions, as well as an MS qualification or a willingness to undertake this in the future is essential.
We will consider applications for a Band 5-6 development role, and can provide you with the opportunity to develop specialist knowledge in MS care, work alongside experienced clinicians, and make a meaningful difference to the health and wellbeing of patients within your local community.
The Band 6 MS Nurse is responsible for the provision of specialist nursing care for a defined patient/client group, practicing as a core member of the multi-disciplinary team. Providing specialist case management and educational support for patients with Multiple Sclerosis (MS) under the supervision of the Senior Multiple Sclerosis Specialist Nurse Lead, coordinating care with partner agencies and key stakeholders with the aim of promoting well-being, education for patients and healthcare professionals in order to prevent hospital admission by maintaining safe and effective care in the community setting. Report and accountable to the Senior Multiple Sclerosis Specialist Nurse Specialist for North Hampshire.

Main duties, tasks & skills required
- To be competent and confident in discussing treatment options with Senior Multiple Sclerosis Nurse Specialist, medical and other nursing specialists.
- Offer an individual service and psychological support to patients and carers, respecting their diverse cultural and ethnic backgrounds.
- Provide MS education and supervision for Patient Support, Primary Care Teams and GP'S to enable practitioners to deliver evidence-based practice.
- Contribute to the ongoing promotion of the Community MS service to all key stakeholders including patients/carers.
- Assist in the monitoring, evaluation and audit the MS service quality and develop reporting mechanisms in order to meet nationally and locally set targets.
- Contribute to develop clinically effective practice in MS services through care pathways, which are integrated alongside the whole system of primary and secondary care.
- Assist in the maintenance and update of protocols according to national and local guidelines for the safe and effective provision of the MS service
Liaise alongside the Senior Multiple Sclerosis Nurse Specialist with key stakeholders and utilise established networks to ensure consolidation of current good practice and consistency with priorities which include:

Essential
Maintain registration on the Health Professions Council or NMC and comply with the standards for registrants, or Diploma/degree in Occupational Therapy or Physiotherapy.
Current registration with the HCPC
MS Qualification or willingness to undertake.
Relevant post qualification experience.
