At AMG Nursing and Care Services, our work goes beyond delivering care. We support people to regain confidence, independence and dignity following a hospital stay. We are currently seeking caring, driven Reablement Community Care Workers to join our expanding team.

What You’ll Be Doing
As a Reablement Care Worker, you will support individuals who have recently returned home from hospital, helping them to settle safely and comfortably. You will work in partnership with Occupational Therapists and District Nurses, supporting clients throughout their reablement journey.
Your day-to-day will include:
✔ Personal care and wellbeing support
✔ Medication prompts and assistance
✔ Meal preparation
✔ Light domestic tasks
✔ Clear and accurate communication with your team
✔ Offering reassurance, kindness and encouragement
Working Hours
6.00am – 2.30pm
3:00pm – 10:30pm
Hours available during the week, *must be able to commit to working alternate weekends*
You will work locally and support regular clients, giving you the opportunity to build meaningful relationships and be part of their journey back to independence.
What We’re Looking For
· A valid UK driving licence
· Right to work in the UK
· Aged 18 or over
· Previous experience in community care
