As an ACP, you will work as an autonomous practitioner, delivering advanced clinical assessment, diagnosis, treatment and care planning for patients with a wide range of urgent, acute and complex healthcare needs.
Working closely with community nurses, therapists, GPs, ambulance services, acute trusts and wider system partners, you will play a key role in delivering high-quality, responsive care across the Integrated Care System (ICS).
This is a full-time position working 37.5 hours per week across a seven-day service, with flexibility to meet the needs of the service.

Provide clinical expertise and support to members of the multidisciplinary team.
-
Work collaboratively with system partners to ensure seamless patient care and effective discharge and admission avoidance pathways.

Recognise and respond appropriately to deteriorating patients, escalating concerns where necessary.
-
Maintain accurate and contemporaneous clinical records in line with professional and organisational standards.
-
Participate in clinical audit, governance, service development and quality improvement initiatives.
