Overview
We are seeking a dedicated and organised Care Coordinator to join our team, providing essential support within a care home specialising in senior care. The ideal candidate will possess strong leadership and communication skills, with experience supervising care staff and developing personalised care plans. The Care Coordinator will be responsible for overseeing daily operations, ensuring high standards of care, and fostering a compassionate atmosphere for residents and staff alike.
Duties
- Coordinate and oversee the delivery of personalised care plans tailored to each resident’s needs
- Supervise and support care staff, ensuring compliance with care standards and organisational policies
- Develop, review, and update care plans in collaboration with residents, families, and healthcare professionals
- Maintain effective communication with residents, families, healthcare providers, and team members to ensure seamless service delivery
- Organise staff rotas, monitor attendance, and manage workload distribution to optimise team efficiency
- Ensure all care activities adhere to legal requirements, health & safety regulations, and organisational policies
- Assist with training new staff members and provide ongoing leadership to foster a positive team environment
- Utilise IT systems for documentation, scheduling, and reporting purposes
- Support residents’ wellbeing by promoting social activities and maintaining a safe environment
Experience
- Proven supervisory experience within a care home or senior care setting
- Strong leadership skills with the ability to motivate and guide a diverse team
- Experience in developing and managing personalised care plans for elderly residents
- Excellent communication skills, both verbal and written, with the ability to liaise effectively with residents, families, and healthcare professionals
- Driving licence is desirable for travel between multiple sites or community engagement activities
- Competent in using IT systems for documentation, scheduling, and communication purposes
