We are looking for an enthusiastic, organised and compassionate Care Coordinator to join our expanding domiciliary care team.
This is a fantastic opportunity for someone who enjoys working in a fast-paced environment, has excellent organisational skills and is passionate about ensuring people receive safe, high-quality and person-centred care.
The Role
As a Care Coordinator, you will play a key role in coordinating our care services and supporting both our service users and care team.
Your responsibilities will include:
- Coordinating daily care packages and ensuring visits are covered.
- Creating and managing staff rotas and care runs.
- Matching care workers to service users based on their needs and preferences.
- Responding to changes in service user needs and staffing requirements.
- Communicating effectively with service users, families, care workers and professionals.
- Monitoring care calls and addressing missed or late visits.
- Supporting the recruitment, induction and onboarding of care staff.
- Monitoring staff compliance, training and documentation.
- Assisting with care plan reviews and ensuring care plans remain accurate and up to date.
- Dealing with queries, concerns and incidents promptly and professionally.
- Maintaining accurate records in line with company policies and regulatory requirements.
- Participating in an on-call rota
About You
We are looking for someone who is:
✅ Highly organised with excellent attention to detail✅ Confident using IT and care management systems✅ An excellent communicator✅ Able to remain calm and professional under pressure✅ A strong problem solver who can think on their feet✅ Confident managing rotas and changing priorities✅ Passionate about delivering outstanding care✅ Able to work independently while also being a strong team player✅ Experienced within domiciliary care or a similar care environment
Previous care coordination or scheduling experience is desirable.
