Job Overview
We are seeking a compassionate and dedicated Care Manager to lead and coordinate personalized care services for our senior residents. Duties
- Develop, implement, and regularly update individualized care plans tailored to each resident’s unique needs and preferences
- Coordinate multidisciplinary care teams to deliver comprehensive services, including medication administration, behavioral management, and medical treatments
- Monitor residents’ health status, respond promptly to changes, and communicate effectively with healthcare providers
- Provide guidance and support to caregiving staff on best practices in senior care, dementia management, and assisted living procedures
- Ensure safety protocols are followed diligently, including heavy lifting techniques and emergency response procedures such as first aid
- Oversee medication management, ensuring proper storage, documentation, and adherence to prescribed regimens
- Facilitate engaging activities that promote mental stimulation, social interaction, and overall well-being for residents
Requirements
- Proven experience in nursing home or care home settings with a focus on senior care or Alzheimer’s/dementia care
- Knowledge of medical terminology, case management practices, and hospice or palliative care principles
- Valid certification in first aid; additional certifications in caregiving or nursing are highly desirable
- Strong driving skills with a valid license for transportation responsibilities if required
- Ability to perform physical tasks including heavy lifting and assisting residents with mobility needs
- Excellent communication skills to collaborate effectively with residents’ families, healthcare professionals, and team members
- Demonstrated ability to develop detailed care plans and manage complex behavioral or medical situations effectively
