At My Homecare Gloucester, we support people to live safely, independently, and comfortably in their own homes. Our compassionate team provides high-quality home care, specialist care, live-in care, and 24-hour support to individuals and families across the local community.
Due to continued growth, we are looking for an experienced Care Coordinator to join our friendly team and work as part of our Care Management Team to ensure the smooth running of our domiciliary care service.

The Role
This role is a mixed being both office based, with field based activities such as competency checks, spot checks, assessment, reviews, on the job training along some community visits delivering frontline care support when needed.
You will play an important role in ensuring our clients receive safe, person-centred care while supporting our care team to deliver excellent service.
Key responsibilities include:
- Coordinating care visits and supporting the day-to-day running of the service
- Undertaking initial assessments for new clients
- Writing and reviewing care plans and risk assessments
- Conducting care plan reviews with clients and families
- Ensuring carers understand and follow care plans
- Supporting and supervising members of the care team
- Helping arrange cover for staff sickness and annual leave
- Participating in the on-call rota to support the team when needed
- Occasionally supporting with frontline care delivery
What We Are Looking For
We are looking for someone who is organised, compassionate, and passionate about delivering high-quality care.
The ideal candidate will be confident supporting staff, building relationships with clients and families, and ensuring care services run smoothly.
Essential requirements:
- Level 3 Diploma in Health & Social Care (or equivalent)
- Minimum 2 years’ experience in a supervisory role within domiciliary care
- Good understanding of CQC regulations and care standards
- Strong communication and organisational skills
- Ability to lead and support a team
- Full UK driving licence and access to a vehicle
A willingness to work towards Level 5 Diploma in Leadership for Health & Social Care would be advantageous.
Working Pattern
- Full time or Part time - minimum of 20 hours per week
- Combination of office work and community visits
- Participation in on-call rota
- One weekend on / one weekend off supporting care delivery when required
