The Role
We are seeking a dedicated and empathetic Community Support Worker to join our growing team. In this role, you will provide practical and emotional support to individuals in their homes and within the community, helping them achieve their personal goals and maintain independence.
We require the candidate to be able to relocate to different areas according to the company's needs>
Key Responsibilities
- Provide person-centred support in line with individual care plans
- Assist with daily living activities (meal preparation, personal care, household tasks)
- Support clients to access community activities and appointments
- Promote independence, inclusion, and wellbeing
- Maintain accurate documentation and records
- Communicate effectively with families, healthcare professionals, and team members
About You
- Compassionate, patient, and respectful approach
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexible and reliable
- Previous experience in community care or support work (desirable but not essential)
- Relevant qualifications (e.g., Certificate in Health & Social Care) – or willingness to obtain
- Valid driver’s licence ( Required)
