Overview
We are seeking a dedicated and compassionate CHC Nurse Assessor to join our healthcare team. The successful candidate will play a vital role in evaluating patients' health needs to determine eligibility for Continuing Healthcare funding. Duties
- Conduct comprehensive assessments of patients’ health status, focusing on physical, psychological, and social care needs
- Review medical records and liaise with healthcare professionals to gather relevant information
- Evaluate patients’ eligibility for Continuing Healthcare funding based on established criteria
- Prepare detailed reports documenting assessment findings and recommendations
- Communicate assessment outcomes clearly and sensitively to patients and their families
- Maintain accurate, up-to-date documentation in accordance with organisational policies and data protection regulations
- Participate in multidisciplinary team meetings to discuss complex cases and share insights
- Ensure compliance with all relevant clinical guidelines, policies, and legal requirements during assessments
Experience
- Proven experience in patient care within a clinical setting, demonstrating strong clinical judgement
- Knowledge of human anatomy and the ability to apply this understanding during assessments
- Previous experience conducting health assessments or working within a healthcare evaluation environment is desirable but not essential
- Excellent organisational skills with the ability to manage multiple cases efficiently
- Compassionate communication skills, capable of engaging with patients from diverse backgrounds
- Ability to work independently whilst collaborating effectively within a multidisciplinary team
