Coordinate, monitor and review complex packages of care, ensuring services remain responsive, effective and focused on achieving positive outcomes.
Work collaboratively with local authorities, provider organisations, specialist nursing teams and community services to deliver integrated care.
Support safeguarding processes and contribute to risk assessment and risk management planning.
Facilitate and lead multidisciplinary meetings, case conferences and care reviews.
Monitor the quality and effectiveness of commissioned services and identify opportunities for service improvement and innovation.
Promote best practice in learning disabilities care and contribute to the ongoing development of Continuing Healthcare services.
Support the organisation's commitment to reducing health inequalities and improving outcomes for people with learning disabilities and their families.
