We are recruiting for a Healthcare Assistant to join our new Rotherham Identification and Support Service.
Salary: Marie Curie Clinical Pay Scale, aligned with AFC 2 £25,272 pro rata per annum
Hours: Part Time 22.5 hours per week – Monday to Friday – 8 am – 4 pm
Contract Type: Permanent
Based: Woodside Community Hospital, Moorgate Road, Rotherham S60 2TZ

This service will be based in Primary Care within Rotherham, where you will use a predictive analytic software tool used by the Rotherham NHS trust to search for patients who are not yet identified as having palliative care but are likely to be in the last year of life. You will be the initial contact with patients, offering support and the opportunity to complete advance care planning, and signposting to other services. You will visit patients in their own homes and refer them to either hospice, voluntary or community services.
With experience of palliative care, you're ready and able to make a real difference. You will work in close collaboration and under the line management of the Clinical Nurse Specialist in the service to provide direct support to patients and their families who are newly identified as having a palliative diagnosis. The role will include making first contact, arranging follow up and offering signposting to support services and advance care planning discussions so excellent communication skills are a must. The role will also involve visiting local community hubs to provide awareness, education and information on advance care planning.
What we’re looking for:

S/NVQ Level 2/3 qualification or an equivalent certificate/diploma is required

Relevant community experience, providing care and support for people with life limiting illness or palliative care needs

Advanced communication skills with the ability to have difficult and sensitive conversations.

Emotional resilience and the ability to work on your own are crucial, though we will of course provide excellent on-going training and support

Be a UK licensed driver with access to a car and business insurance is essential
